The Del Corazon Hospice Care Team delivers care wherever the patient calls home.  Hospice services are brought to our patients in a:

• Private residence
• Retirement community
• Long-term care facility (including nursing homes, group homes and other assisted living environments)
• Rehabilitation facility

While hospice care is usually delivered to the home, sometimes there are times when a patient’s condition requires them to be in a hospital, extended-care, or rehabilitation facility.  No matter where our patients live, they are entitled to the same hospice benefits and the care team continues to be involved in the care.  There is always someone on call to help with whatever issues might arise.  Our hospice care assures you and your family that you are not alone.  Once enrolled in hospice, you are entitled to the same services no matter where you are.
